Contest Rules

Captivating Aesthetics is committed to maintaining the security and confidentiality of personal information in accordance with applicable privacy legislation and our Privacy Policy. By completing and signing this form, you are consenting to Captivating Aesthetics collecting, using and disclosing your personal
information in order to identify and communicate with you, for such other purposes as may be necessary in order to provide you with the products and/or services you have requested, and for any other purposes where you consent or where such collection, use or disclosure is permitted or required by law. You represent that you have all necessary authority and/or have obtained all necessary consents from any other individuals about whom you have disclosed personal information to CA in order to enable us to collect, use and disclose such personal information to fulfill the purposes described above. RULES:

1. To enter and qualify, print your name, address, telephone number, age and email
   address, and deposit in the draw box.
2. Limit of one entry per person per day. Additional entries will result in disqualification.
3. No purchase is necessary to enter the contest.
4. Contest is open to all persons except the employees and their immediate families of
   Captivating Aesthetics.
5. All entries become property of CA. We reserve the right to publish the name and a
   photograph of the winners for publicity purposes and you hereby consent to
   such publications.
6. The prize must be accepted as offered, no substitutions or cash equivalent being
   permissible. Prizes are non–refundable and non-transferable.
7. Contest is subject to all Federal and Provincial regulations.
8. Prize winner is responsible for any federal or provincial taxes as a result of wining the prize.
9. Winners will be chosen by random draw of all entries at the published closing time
   of the contest.
